COLLEGE INTERCOLLEGIATE ATHLETIC WAIVER FORM
RELEASE, WAIVER AND DISCHARGE
Your
acceptance confirms that you have read, understood and accepted
this release, waiver and discharge. In consideration on my
participation in the Durham College Intercollegiate Athletic
Program and any related tryouts, exhibition, OCAA and CCAA
participation, I hereby release, waiver and discharge the
Board of Governors of UOIT and Durham College, Ontario Colleges
Athletic Association and the Canadian Colleges Athletic Assocation
and its officers, employees and voluntary officials, of and
from all claims, demands, damages, costs, actions, and causes
of action, in respect to death, injury, loss of damage to
my person or property, arising from the participation in the
Durham College Intercollegiate Athletic Program or use of
campus facilities.

Collection
and Use of Personal Information: I hereby, give consent to
UOIT and Durham College to utilize information gathered from
my academic record and my student file soley for the purpose
of assessing my academic progress and intercollegiate athletic
eligibility. I further acknowledge that any information gathered
may be used for statistical purposes.
Applicants
Declaration: I have given complete and true information. I
have considered, understood and agree to the policies and
procedures of the Athlete Academic Success Program. I also
agree that the Athletics Department may release my name and
sports pictures to the media or use them in various promotional
publications, including the display of such on website applications
for the OUA, CCAA, OCAA and on campus. Lastly, I agree to
return any uniforms, equipment, etc. issued to me, in the
condition that I received them. In the event that I lose a
uniform, etc, I will reimburse the College the replacement
cost.
